The vacuum leak is your cause. Be sure you actually fixed it. If either end of that EGR tube is not sealed completely, you will have a vacuum leak.

The LTFT numbers are what I'm interested in and, as expected, they too confirm your lean condition. The "25" represents the maximum positive value that the PCM/ECU can adjust the trim. To have such a high number indicates that too much oxygen/not enough fuel is getting to the combustion chambers and the PCM/ECU cannot add anymore fuel to compensate. If it were a high negative number, the opposite would be the case, i.e. too rich. New EGR seals/gaskets are quite cheap. Replace them at both ends and recheck. It's highly unlikely that you found a vacuum leak AND there's still a problem. The sensors simply did their jobs and exposed the leak.

Leave your battery disconnected for a bit too when you fix things, force the LT and ST trims to reset, hopefully it will fix it.
